Referral pathway for healthcare providers

Refer a patient located in Ontario or British Columbia when focused assessment of fertility, menstrual or ovulatory health, or male reproductive health would help move the care plan forward. Asali currently accepts referrals from regulated healthcare professionals only.

Who may benefit?

  • Patients without timely access to a primary-care provider
  • Patients needing clinically indicated conventional laboratory assessment
  • Women or men needing evidence-based fertility or preconception assessment
  • Heavy, prolonged, irregular, or absent periods
  • Suspected PCOS, anovulation, endometriosis, fibroids, or other pelvic concerns
  • Male fertility concerns, semen assessment, or symptoms suggestive of testosterone deficiency
  • Medication, supplement, or anabolic-steroid exposure that may affect reproductive health
  • Preconception medication or medical-condition review
  • Patients who may meet criteria for fertility-specialist or gynecology referral
  • Under 35: no conception after 12 months of regular unprotected intercourse
  • Age 35 or older: no conception after 6 months
  • Over 40: more immediate evaluation may be warranted
  • At any age: known or suspected conditions associated with infertility, including irregular or absent cycles, PCOS/anovulation, endometriosis, uterine or tubal disease, suspected male factor, or diminished ovarian reserve risk
Final referral decisions depend on the clinical assessment and the receiving service's criteria and availability.

Referral received

Clinical question, relevant history, and prior results arrive by secure fax, using the Asali referral form when available.

Patient contacted

The patient may provide clarification through secure asynchronous messaging based on their preference. If the NP requires further information, a short video appointment is arranged.

Shared clinical review

The patient's history and preferences, the clinical question, and the referring provider's recommendations are considered through an independent NP assessment.

Orders placed

Clinically indicated standard laboratory or imaging requisitions are arranged.

Results communicated

The patient and referring provider receive a consultation letter and recommended medical next steps.

Patient returns

The patient returns to the referring provider for ongoing care; specialist referral proceeds when indicated.

Referrals must come from a regulated healthcare professional and are accepted by secure fax only. Patient self-referral is not available at this stage. Ordinary email is for general questions and must never include names, health-card information, clinical records, or other personal health information.
